We are seeking a skilled Physical Therapist to join our team at Sky View Rehabilitation and Health Care Center. As a Physical Therapist, you will play a vital role in helping our residents achieve their highest level of physical, mental, and social well-being.
Job Summary:The Physical Therapist will be responsible for planning, developing, and implementing physical therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations. This includes creating individualized therapy plans, conducting resident assessments, and supervising Physical Therapy Assistants.
Responsibilities:- Plan, develop, and implement physical therapy services to meet the needs of our residents.
- Conduct resident assessments and create individualized therapy plans to help residents achieve their goals.
- Supervise Physical Therapy Assistants and provide guidance and support as needed.
- Maintain accurate and up-to-date treatment records and resident files.
- Participate in discharge planning and development of resident care plans.
- Current license and registration to practice as a Physical Therapist in the State of NY.
- Current malpractice insurance and license in good standing.
- Dedicated to providing compassionate care and committed to quality outcomes.
- Excellent communication and customer service skills.
- Ability to work well under pressure and in a team environment.
