We are seeking a highly skilled and dedicated Physical Therapist to join our team at Sky View Rehabilitation and Health Care Center. As a Physical Therapist, you will play a vital role in helping our residents achieve their highest level of physical, mental, and social well-being.
Job Summary:The Physical Therapist will be responsible for planning, developing, and implementing physical therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations. This includes creating individualized therapy plans, conducting resident assessments, and supervising Physical Therapy Assistants.
Responsibilities:- Plan, develop, and implement physical therapy services to meet the needs of our residents.
- Conduct resident assessments to identify areas for improvement and develop individualized therapy plans.
- Supervise Physical Therapy Assistants to ensure high-quality care.
- Interpret and instruct physical therapy policies and procedures to personnel, residents, and family members as necessary.
- Participate in discharge planning and development of resident care plans.
- Maintain treatment records, resident files, and progress notes as required.
- Current license and registration to practice as a Physical Therapist in the State of NY.
- Current malpractice insurance and license in good standing.
- Dedicated to providing extraordinary compassionate care.
- Energetic, enthusiastic, committed, and passionate about quality care.
- Genuinely interested in helping ill, injured, disabled, elderly, emotionally upset, and sometimes challenging residents.
- Patience, a cheerful disposition, and willingness to assist any residents and co-workers.
- Excellent communication, customer service, and organization skills.
- Trusted to work independently and in a team environment.
- Able to work well under pressure, flexible, and will work harmoniously with all staff.
