Clinical Assessment Advisor, Home Health Specialist

At CenterWell Home Health, we're dedicated to providing personalized, comprehensive home care for patients managing chronic conditions or recovering from injury, illness, surgery, or hospitalization. As a Clinical Assessment Advisor, you'll play a vital role in our care teams, working alongside nurses, physical therapists, occupational therapists, speech-language pathologists, home health aides, and medical social workers to help patients rehabilitate, recover, and regain their independence.
Key Responsibilities:- Conduct initial and ongoing evaluations of patient needs, ensuring accurate assessments and effective care plans.
- Develop and implement comprehensive care plans, incorporating teaching related to health maintenance, prevention, and safety.
- Coordinate available resources to manage care plans, ensuring stated outcomes are achieved.
- Periodically reassess patient needs and revise care plans as necessary.
- Promote communication between team members, attending physicians, administrative staff, referral sources, and external case managers to ensure appropriateness of care.
- Bachelor of Science degree in Nursing (BSN) preferred.
- At least 2 years of clinical home care experience, with a minimum of 1 year in a clinical management role.
- Current CPR certification.
- Excellent verbal and written communication skills.
- Excellent interpersonal skills.
- Knowledge of state and federal home health agency regulations and compliance standards.
- Knowledge of clinical policies and procedures and ability to implement.
- Knowledge of clinical structure of PDGM.
- Must read, write, and speak fluent English.
- Must have good and regular attendance.
- Approximate percent of time required to travel: 60%.
