Job Summary

We are seeking a skilled Clinical Case Manager LVN/RN to join our team at EQUANIMITY HOME HEALTH SERVICES. In this role, you will work closely with patients, healthcare providers, and community resources to deliver high-quality care and promote positive patient outcomes.

About the Position:

  • Coordinate home care services for patients, ensuring timely and effective care transitions.
  • Develop and implement individualized care plans, prioritizing patient needs and goals.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.

Requirements:

  • Minimum 2 years' experience as a Clinical Case Manager LVN/RN.
  • Current licensure as a RN/LVN in the State of CA.
  • Experience with OASIS-E and EMR charting systems.

Salary: $45.00 - $65.00 per hour

Expected Hours: 35 hours per week

Schedule: Monday to Friday
