**Key Responsibilities:**



* Provide safe and effective implementation of direct care in accordance with established policies, procedures, and standards of care.



* Administer medical treatment, including medications, as prescribed by the physician.



* Develop and implement individualized treatment plans that promote recovery and well-being.



* Collaborate with multidisciplinary teams to ensure comprehensive care and support.



* Provide education and support to individuals and their families to promote recovery and independence.



* Participate in quality improvement initiatives to enhance services and outcomes.

**Requirements:**



* Graduation from an accredited LVN/LPT program and current licensure.

* Bilingual Spanish language skills.
