**About the Role**

We are seeking a skilled RN Clinical Care Manager to join our team at UnitedHealth Group. This role is responsible for engaging members face-to-face and/or telephonically to complete a comprehensive needs assessment, including assessment of medical, behavioral, functional, cultural, and socioeconomic needs.

In this position, you will develop and implement person-centered care plans to address needs including management of chronic health conditions, health promotion and wellness, social determinants of health, medication management, and member safety in alignment with evidence-based guidelines.

You will partner and collaborate with internal care teams, providers, and community resources/partners to implement care plans and make appropriate referrals based on care integration needs.

This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

We offer a competitive salary of $85,000 per year, plus benefits, including medical, dental, and vision insurance, 401(k) matching, and paid time off.

Requirements

  • Unrestricted RN license in the State of Louisiana
  • RN with 2+ years of nursing experience
  • Intermediate level of computer proficiency (including Microsoft Outlook) and ability to use multiple web applications and enter/retrieve data in electronic clinical records
  • Ability to travel locally approximately 25% of the time and up to 100 miles round trip with reliable transportation, that will enable you to travel to client and/or patient sites within a designated area, with a current and non-restricted state of Louisiana driver's license and state-required insurance
