Role Overview

We are seeking an experienced RN to join our team as a Case Manager. The successful candidate will be responsible for managing patient and family caseloads, including assessment, eligibility determination, admission processing, planning, implementation, and evaluation phases of the nursing process.

Key Responsibilities

Some of the key responsibilities of this role include:

  • Assessing patient physical, psychological, social, and spiritual factors that may impact patient and family needs.
  • Communicating with attending physicians, interdisciplinary teams, and other agencies to coordinate optimal care.
  • Reassessing patient needs and updating plans of care regularly.
  • Maintaining accurate patient records and documentation.
