Job Overview

We are seeking an energetic and self-driven Mental Health Case Manager and Community Support Specialist to join our team. This role involves connecting participants to community resources, assisting with skill building, and preventing transition to a higher level of care.

Key Responsibilities
  1. Create a safe and trusting environment to build rapport with participants and their designated supports.
  2. Complete assessments, treatment plans, reviews, and progress notes in a timely manner.
  3. Foster effective collaboration and communication with treatment providers and staff.
  4. Maintain confidentiality of verbal and written communication.
  5. Adhere to HSI policies and procedures as well as the NASW Code of Ethics.
  6. Navigate Electronic Medical Record systems (Athena) efficiently.
  7. Participate in ongoing education and training, including 10 CEUs annually.
Compensation and Benefits

This full-time position offers a competitive salary ranging from $20.00 to $24.50 per billable hour, determined by licensure and experience.
