EverHeart Hospice is seeking a compassionate and skilled Care Center RN to join our team.
About the RoleWe are looking for a dedicated and experienced RN to provide high-quality patient care in our Inpatient Care Center. As a Care Center RN, you will be responsible for delivering direct patient care, working closely with our interdisciplinary team to develop and implement individualized care plans.
Responsibilities- Provide skilled nursing care to patients, including symptom management, emotional support, and education.
- Collaborate with the healthcare team to develop and implement patient care plans.
- Conduct initial nursing assessments and document patient information accurately.
- Order and manage durable medical equipment as needed.
- Work evenings and weekends as needed to meet patient needs.
- Complete all documentation as required by the organization.
- Graduation from an approved School of Nursing.
- Current RN licensure in the State of Ohio and eligibility to obtain licensure in the State of Indiana or current RN licensure in the State of Indiana and eligibility to obtain licensure in the State of Ohio.
- Minimum of two years of experience in Medical-Surgical, Oncology, or community health nursing.
- Hospice experience preferred.
