Position Overview:

We are looking for a dedicated and experienced Permanent Resident Care Manager (RCM) to join our vibrant healthcare team.

The successful candidate will oversee the comprehensive healthcare needs of our residents, ensuring their well-being and comfort.

This position requires a compassionate individual with a profound understanding of resident care management and a strong dedication to enhancing the quality of life for our residents.

The ideal candidate will possess critical thinking abilities, outstanding organizational and leadership skills, and a passion for delivering exceptional patient care.


Key Responsibilities:

  • Design, implement, and evaluate personalized care plans for each resident, tailored to their specific healthcare requirements and preferences.
  • Coordinate and supervise the activities of the healthcare team to ensure effective execution of all care plans.
  • Conduct regular evaluations of each resident's health condition, making necessary modifications to their care plans as needed.
  • Maintain precise and current records of each resident's care, including medical history, treatments, and progress.
  • Establish and uphold effective communication with residents, their families, and healthcare team members to ensure all parties are informed about the resident's health and care.
  • Advocate for residents' health and welfare, ensuring their needs and preferences are acknowledged and fulfilled.
  • Collaborate with other healthcare professionals and departments to provide comprehensive, integrated care.
  • Adhere to all relevant regulations, standards, and policies regarding resident care management.
  • Provide leadership and direction to the healthcare team, fostering a positive and supportive work environment.
  • Engage in ongoing training and professional development activities to stay current with the latest advancements in resident care management.

Qualifications:

  • Bachelor's degree in Nursing, Healthcare Administration, or a related field. A Master's degree is a plus.
  • At least 5 years of experience in resident care management, preferably in a long-term care environment.
  • Demonstrated experience in creating and implementing effective care plans.
  • Thorough knowledge of resident care management principles and best practices.
  • Exceptional leadership and organizational abilities, with the capacity to coordinate and oversee a healthcare team.
  • Excellent communication and interpersonal skills, with the ability to build and maintain effective relationships with residents, their families, and team members.
  • Strong advocacy skills, committed to promoting residents' health and welfare.
  • Proficiency in utilizing healthcare information systems and software.
  • A current, valid license to practice as a Registered Nurse or equivalent.
  • A commitment to continuous professional development and learning.
