We are seeking an experienced Palliative Medicine Physician to join our team at PeaceHealth. As a Hospice Physician, you will play a vital role in providing compassionate care to patients and their families during the advanced stages of illness.
Key Responsibilities- Provide medical, spiritual, and emotional support to patients and their families
- Collaborate with a multidisciplinary team to develop and implement individualized care plans
- Consult with other physicians and healthcare professionals to ensure seamless care coordination
- Participate in quality improvement initiatives to enhance patient outcomes and experiences
- Maintain compliance with regulatory requirements and industry standards
- Board certification in Palliative Medicine or Hospice and Palliative Medicine
- Experience in hospice and palliative care, with a focus on patient-centered care
- Strong communication and interpersonal skills, with the ability to work effectively with diverse stakeholders
- Ability to travel throughout Clark and Cowlitz Counties
