We are seeking a highly skilled Medical Assistant to join our team as a Patient Care Coordinator for the Care Coach Connect Program. This is an exciting opportunity to work in a dynamic environment where you will have the chance to make a real difference in the lives of our patients.
Job SummaryThis role requires a strong understanding of patient care and a passion for delivering exceptional service. As a Medical Assistant, you will be responsible for coordinating patient care activities, organizing clinical environments, and providing support in patient care situations.
Key Responsibilities:- Coordinate healthcare screening results as directed by providers and company policies
- Travel with providers to patient homes, mobile units, and other locations within the NTX Market and surrounding areas
- Perform quality control checks on equipment and maintain accurate records
- Assist in development of Plan of Care paperwork for assigned Advanced Practice Clinicians
- High school graduate or GED equivalent
- Nationally recognized Medical Assistant certification or registration
- CPR certification
- Basic computer literacy
$45,000 - $65,000 per year, depending on experience
