About This Job

The Primary Care Registered Nurse is a vital member of the primary care multi-disciplinary team, serving a diverse patient population with a range of health conditions.

Job duties may include telephone triage, nursing visits, medication administration, vaccine management, and specific office-based procedures based on policies and protocols.

Responsibilities
  • Telephone triage and nursing visits
  • Medication administration and vaccine management
  • Office-based procedures based on policies and protocols
  • Collaboration with the primary care multi-disciplinary team
  • Providing patient-centered care and education
  • Documenting advanced care goals in the EHR
  • Facilitating the advanced care planning process
  • Team-based care and coordination
Requirements
  • Graduate of an approved school of nursing with current registration in Massachusetts
  • Bachelor of Science Degree in Nursing required for newly licensed nurses
  • Bilingual Spanish preferred
  • At least 3 months recent telephone triage experience required
  • Ambulatory Care experience required (surgical/procedural excluded)
Preferred Qualifications
  • Triage experience in Primary Care, Urgent Care, or Community Health nursing
  • Recent Primary Care experience preferred
  • Excellent critical thinking skills and utilization of triage protocols
  • Technical dexterity and ability to work with various information systems and databases
