Role Overview: In this position, you will collaborate closely with primary care physicians to ensure exceptional care for a varied patient demographic. Work alongside multidisciplinary teams to enhance health outcomes and minimize unnecessary healthcare utilization for high-risk individuals.

Key Responsibilities:

  • Engage with patients at the point of care to evaluate health and risk profiles
  • Implement early interventions for condition management and medication compliance
  • Coordinate with care teams to facilitate seamless care
  • Encourage advance care planning and assist with patient navigation
  • Monitor patients after discharge to mitigate readmission risks
  • Create patient-centered care plans through collaborative decision-making
  • Identify and address barriers to care and social determinants affecting health

Required Skills:

Qualifications:

  • At least 5 years of clinical nursing experience
  • Valid Registered Nurse license in the applicable state
  • Preferred experience in care management
  • Comfortable with technology and electronic health record systems
