As a Licensed Practical Nurse (LPN) on our Outpatient Services team, you will play a vital role in delivering high-quality patient care in a fast-paced and dynamic environment. You will work collaboratively with our interdisciplinary team to provide patient-centered care that meets the unique needs of each individual.

Responsibilities
  • Provide direct patient care under the supervision of a licensed healthcare professional, including administering medications, performing vital signs, and providing wound care.
  • Conduct thorough patient assessments, develop and implement individualized care plans, and evaluate patient outcomes.
  • Collaborate with other healthcare professionals to coordinate patient care and ensure seamless transitions between departments.
  • Maintain accurate and detailed patient records, including medical histories, treatment plans, and progress notes.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures that promote excellent patient care.
Requirements
  • Graduate of an accredited LPN program.
  • Current LPN licensure in the state of hire.
  • Basic Life Support (BLS) certification through the American Heart Association.
  • Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
  • Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
