As a Licensed Practical Nurse (LPN) on our Outpatient Services team, you will play a vital role in delivering high-quality patient care in a fast-paced and dynamic environment. You will work collaboratively with our interdisciplinary team to provide patient-centered care that meets the unique needs of each individual.

Responsibilities
  • Provide direct patient care under the supervision of a licensed healthcare professional.
  • Conduct thorough assessments of patients' physical and emotional needs.
  • Develop and implement individualized care plans that address patients' unique needs and goals.
  • Collaborate with other healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Document patient information accurately and thoroughly in accordance with our policies and procedures.
Requirements
  • Graduate of an accredited practical nursing program.
  • Current LPN licensure in the state where nursing duties are performed.
  • Basic Life Support certification through the American Heart Association or successful completion of course within 30 days of hire.
  • Ability to work in a fast-paced environment and prioritize multiple tasks simultaneously.
