About the Role

Aveanna Healthcare is seeking a skilled Private Duty Nurse LVN/LPN to join our team of dedicated healthcare professionals. As a Private Duty Nurse, you will play a vital role in providing high-quality, patient-centered care to our clients in the comfort of their own homes.

Key Responsibilities
  • Implement and manage physicians' prescribed plans of care for patients
  • Conduct thorough patient assessments and coordinate care with other healthcare professionals
  • Administer prescribed medications, treatments, and therapies as ordered by physicians
  • Document care provided in a timely and compliant manner
  • Provide health education and training to patients and their families
  • Accept and write verbal orders from physicians as required, and obtain co-signature of Case Manager on all verbal orders
  • Follow up with physicians, clients, family members, staff, and Case Manager on client problems, concerns, and changes in client condition
Requirements
  • Active, unencumbered license (RN/LPN/LVN) in the state in which the clinician will practice
  • Current CPR certification from American Red Cross, American Heart Association, or BLS
  • TB skin test (current within last 12 months)
  • Six months prior hands-on nursing experience preferred but not required
  • Reliable transportation
