About the Role

We are seeking a highly motivated and compassionate individual to join our team as a Home Based Behavioral Health Support Specialist. In this role, you will work closely with our clinical team to provide in-home support services using an evidence-based model.

Your primary responsibilities will include:

  • Providing case management and support services to clients and their families
  • Serving as a bridge to other service providers and facilitating communication between stakeholders
  • Conducting assessments and implementing treatment plans tailored to each client's unique needs
  • Maintaining accurate records and paperwork to support our treatment model

Requirements

To be successful in this role, you will need:

  • A Bachelor's degree in Social Work, Psychology, or a related human services field
  • Excellent communication and interpersonal skills
  • The ability to work independently and as part of a team

What We Offer
