About the Role

Key Responsibilities
  • Conduct Comprehensive Assessments: Conduct thorough health, self-management, socio-economic, behavioral, and risk status assessments with patients to identify their unique needs and develop personalized care plans.
  • Develop and Implement Care Plans: Establish and maintain comprehensive, high-quality, meaningful care plans based on the documented patient-set goals and needs.
  • Coordinate Care Services: Conduct proactive outreach, schedule calls, and provide appropriate follow-up and documentation according to established policy and procedure.
  • Collaborate with Healthcare Teams: Maintain ongoing communication with specialty care physicians and other providers involved in the patient's plan of care, updating the care plan as necessary.
  • Promote Quality Improvement: Participate in quality improvement projects as needed and implement quality improvement initiatives to enhance patient outcomes.
  • Foster Effective Communication: Promote effective collaboration amongst care team members, including the patient and family, and identify and build upon the assets and strengths of all members of the care team.
  • Utilize Technology: Effectively utilize all tools available to support care coordination, maintaining a high level of proficiency with tools within electronic medical records (EMRs) and other applications intended to support patient care.
Requirements
  • Education: Graduate of an accredited RN program or graduate of an accredited LPN program.
  • License and/or Certification: Ohio Registered Nurse Licensure or Ohio Licensed Practical Nurse, BLS Certification.
  • Skills: Excellent interpersonal, written, and verbal communication skills, demonstrated ability to work collaboratively, ability to meet deadlines, prioritize, organize time, and work autonomously.
Preferred Qualifications
  • Education: BSN preferred.
  • Skills: Care management experience preferred, experience with quality improvement processes preferred.
  • Years of Experience: 2+ years of experience.
