Job Summary

We are seeking a highly skilled Social Worker to join our team at ProMedica Health System. As a Social Worker, you will play a critical role in coordinating patient care and ensuring the best possible outcomes for our patients.

Key Responsibilities
  • Conduct initial assessments and develop comprehensive discharge plans for patients
  • Collaborate with medical staff and continuing care coordinators to ensure seamless patient care
  • Provide education and support to patients and families regarding discharge planning and facility placement
  • Communicate effectively with community mental health centers and other healthcare providers
  • Document all interactions with patients and families in a timely and accurate manner
Requirements
  • Bachelor's degree in Social Work or related field from an accredited institution
  • Three to five years of experience in a comprehensive inpatient psychiatric unit or comparable work experience
  • Current licensure as a Licensed Social Worker (L.S.W.), Master of Social Work (MSW), or Professional Counselor from the State of Ohio
Preferred Qualifications
  • Certification in Nonviolent Crisis Intervention
