Discharge Planning Coordinator

The Discharge Planning Coordinator at ProMedica Health System plays a vital role in ensuring a seamless transition of care for patients who are psychosocially complex, have Social Determinants of Health (SDOH) needs, or require assistance with transitions of care or discharge planning.

In this position, you will intervene with patients and caregivers, offering supportive interventions and coordinating the development of a discharge plan of care for high-risk/complex patient populations.

You will collaborate with the care team, including Physicians, Nurses, RN Acute Care Navigators, and Contracted Vendors, to ensure that patients receive exceptional care and avoid unnecessary delays in care or discharge.

Key responsibilities include assessing patient and caregiver psychosocial risk factors, intervening with emotional, social, and financial consequences of illness and/or disability, and advocating for patient and caregiver empowerment and independence.

Additionally, you will develop therapeutic relationships, obtain psychosocial and SDOH information, and identify patients most at risk for readmission without intensive discharge planning.

The ideal candidate will have knowledge of regulations, requirements, and community resources, as well as experience in patient advocacy, care management, and knowledge of hospital and community resources.

Requirements:

  • Knowledge of regulations, requirements, and community resources
  • Experience in patient advocacy, care management, and knowledge of hospital and community resources
  • Specialty certification in care management (CCM, ACM, or similar)
