We are currently looking for a dedicated Family Medicine Nurse Practitioner. In this role, you will provide exceptional healthcare services to patients across various age groups, addressing a wide array of health concerns, injuries, and disabilities. Your key responsibilities will include performing comprehensive physical assessments, referring patients to specialists as necessary, conducting wellness visits, and prescribing medications tailored to individual health needs.

Key Responsibilities:
  • Deliver thorough primary care services to patients of all ages, focusing on diagnosing and managing both acute and chronic health conditions while promoting preventive care.
  • Conduct detailed medical evaluations, which encompass gathering comprehensive medical histories, executing physical examinations, ordering and interpreting diagnostic tests, and establishing accurate diagnoses.
  • Formulate and implement evidence-based treatment strategies for patients, which include prescribing medications, advising on lifestyle changes, and coordinating referrals to specialized care when required.
  • Work collaboratively with multidisciplinary teams, including nurses, physicians, and behavioral health specialists, to ensure integrated and comprehensive patient care.
  • Provide education to patients regarding health promotion and disease prevention, emphasizing lifestyle changes, vaccinations, screenings, and early detection of health issues.
  • Maintain precise and current electronic medical records (EMR) and documentation in compliance with community health center standards and regulatory requirements.
  • Facilitate effective communication and coordination of care among patients, addressing inquiries and enhancing patient engagement.
  • Ensure timely management of patient interactions and correspondence.
  • Deliver culturally competent care to diverse patient populations, honoring their cultural, religious, and social beliefs while addressing health disparities.
  • Advocate for patients' rights, assisting them in navigating the healthcare system, including securing insurance coverage and accessing community resources.
  • Engage in quality improvement initiatives, such as performance enhancement projects and clinical audits, to elevate the quality and safety of patient care.
  • Comply with all relevant federal, state, and local regulations, including community health center policies, HIPAA, and OSHA guidelines.
  • Participate in community outreach and health education initiatives to promote public health and wellness.

Knowledge, Skills & Abilities:
  • Comprehensive understanding of available services at Third Street, community resources, and effective communication strategies.
  • Proficient in both written and verbal communication, interviewing techniques, managing multiple tasks, and problem-solving.
  • Able to prioritize tasks efficiently, exercise sound judgment, and maintain confidentiality while managing patient information.

Qualifications:
  • Current APRN license in Ohio
  • Board Certified or a letter indicating eligibility for certification with the expectation of obtaining certification within one year.
  • Valid Driver's License
