About This Opportunity

We are seeking a compassionate Patient Advocate to join our team at Notre Dame Long Term Care Center. As a Patient Advocate, you will have the opportunity to make a meaningful difference in the lives of our residents, providing them with emotional support, companionship, and personalized care.

Estimated Salary: $50,000 - $60,000 per year, depending on experience and qualifications.

Responsibilities
  • Develop and implement personalized care plans for each resident
  • Conduct regular assessments and updates to ensure residents' needs are being met
  • Collaborate with healthcare professionals to coordinate medical care and services
  • Provide education and support to residents and families on topics such as nutrition, medication management, and disease prevention
  • Facilitate social interactions and activities to promote residents' mental and physical well-being
  • Document and report any changes in resident condition or behavior
  • Participate in quality improvement initiatives to enhance resident care and satisfaction
Requirements
  • Minimum one year of experience in a healthcare setting, preferably with older adults or individuals with disabilities
  • High school diploma or equivalent required; degree in a related field (e.g. nursing, social work) preferred
  • Valid driver's license and reliable transportation
  • Ability to lift 20 pounds or less, with frequent bending, stooping, and standing
  • Ability to work flexible hours, including evenings and weekends, as needed
  • Ability to pass a background check and obtain any necessary certifications or clearances
