Job Summary

We are seeking a highly skilled and compassionate Patient Care Advocate to join our team at Saint Vincent Hospital.

About the Role

The Patient Care Advocate is responsible for facilitating care along a continuum through effective resource coordination to help patients achieve optimal health, access to care, and appropriate utilization of resources.

Key Responsibilities
  • Conducts complex psycho-social assessment and intervention to promote timely throughput, safe discharge, and prevent avoidable readmissions.
  • Integrates national standards for case management scope of services including: Transition Management promoting appropriate length of stay, readmission prevention, and patient satisfaction; Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care.
  • Compliance with state and federal regulatory requirements, TJC accreditation standards, and Tenet policy.
  • Provides education to physicians, patients, families, and caregivers.
Requirements

To be successful in this role, you will need:

  • A Master's degree in Social Work (MSW) from an accredited institution.
  • At least 2 years of experience in acute hospital settings.
  • Certification as an Accredited Case Manager (ACM) is preferred.

Salary Information

The estimated annual salary for this position is $85,000 - $105,000, based on experience and qualifications.
