Case Manager

We are seeking a skilled Case Manager to join our team at Grafton Integrated Health Network. As a Case Manager, you will play a critical role in ensuring the smooth transition of clients through our programs, providing advocacy and support to clients and their families, and coordinating services to meet their unique needs.
Key Responsibilities- Facilitate pre-enrollment tasks to prepare for client enrollment
- Facilitate admission intake process and develop treatment plans
- Coordinate multidisciplinary team meetings to monitor client progress and update plans
- Meet and adhere to funding timelines for Medicaid EPSDT/Waiver, Private Insurance, and Single Case Agreements
- Coordinate and facilitate new IEPs and PCPs, and revisions to those documents
- Ensure accurate and proper documentation for each client's record
- Manage the transition and discharge process, including facilitating discharge from Grafton and transfer to another Grafton program or level of service
- Prepare for and attend FAPT/PCP meetings, and manage the 9-week/quarterly report process
- Respond to stakeholder complaints regarding all aspects of treatment and care for clients
- Master's degree in social work, psychology, or counseling, or Bachelor's degree in social work or psychology with at least one year of documented field experience, or Bachelor's degree with at least three years of experience working with children in a residential facility
- Knowledge of CSA/Medicaid/Private funding processes, services accessible to special needs/mental health populations, and previous case management or like experience
