Job Summary:

The Charge Nurse (LPN/LVN) position is a critical role in our healthcare team, responsible for overseeing the provision of patient care and ensuring the highest practical well-being of our patients.

Key Responsibilities:
  • Participate in and oversee the provision of patient care as prescribed by the attending physician, consistent with accepted standards of care and within the scope of LPN/LVN training and licensure.
  • Provide care directly to patients and/or supervise the direction of this care by a Certified Nursing Assistant to ensure the highest practical well-being is maintained by the patient.
  • Observe, assess, and report patient conditions/changes and document in accordance with Cantex Continuing Care Networks Clinical Policies & Procedures and state/federal guidelines and regulations.
  • Consult with RN and/or Nurse Managers regarding changes in patient's condition.
  • Ensure implementation of care plans as directed and administer medications and treatments as prescribed.
  • Participate in assigning duties and ensure the delivery of appropriate clinical care by the CNA staff.
  • Ensure a safe environment for patient population.
  • Notify attending physician and family or responsible party promptly of any significant change in patient's health condition.
Requirements:
  • Graduation from an accredited Nursing School and a current and valid license in the applicable State.
  • Minimum of one-year supervisory experience in a long-term or healthcare setting is preferred.
  • Ability to read, write, and speak the English language.
