Job Title: LVN Charge Nurse

Regency Integrated Health Services is seeking a skilled LVN Charge Nurse to join our team at Cityview Nursing and Rehabilitation Center in Fort Worth, TX.

Job Summary:

The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This position requires a strong nursing background, excellent communication skills, and the ability to lead a team of caregivers.

Key Responsibilities:
  • Assess and document resident conditions, ensuring accurate and timely reporting to the healthcare team.
  • Implement care plans as directed, prioritizing resident needs and collaborating with the interdisciplinary team.
  • Administer medications as prescribed, maintaining accurate records and adhering to facility policies.
  • Conduct rounds with physicians, recording visits and updates in clinical records.
  • Perform narcotic reconciliation, identifying and addressing any discrepancies.
  • Oversee and monitor aides, providing guidance and support as needed.
  • Initiate and complete incident reports, adhering to facility policies and procedures.
  • Discharge residents according to facility protocols, ensuring a smooth transition.
  • Communicate effectively with residents, families, and healthcare providers, maintaining a positive and respectful attitude.
Requirements:
  • Active Licensed Vocational Nurse (LVN) license in the state of Texas.
  • Completion of an LVN program at an accredited vocational school or college.
  • Experience working in a skilled nursing facility, with a strong understanding of geriatric care and rehabilitation principles.
Physical Demands:
