Job Summary

The Charge Nurse (LPN/LVN) position plays a crucial role in overseeing the provision of patient care, ensuring that it is delivered in accordance with accepted standards and within the scope of LPN/LVN training and licensure.

This position is responsible for providing direct care to patients and supervising the direction of care by Certified Nursing Assistants to maintain the highest practical well-being of patients.

Key Responsibilities
  • Observe, assess, and report patient conditions/changes, documenting in accordance with Cantex Continuing Care Networks Clinical Policies & Procedures and state/federal guidelines and regulations.
  • Provide necessary documentation in the EMR of all information relevant to patient assessment, evaluation, change of condition, in accordance with nursing practices.
  • Consult with RN and/or Nurse Managers regarding changes in patient's condition.
  • Receive, transcribe, and execute physicians' orders using secured and encrypted communication channels in accordance with Company policies.
  • Ensure implementation of care plans as directed and administer medications and treatments as prescribed.
  • Participate in assigning duties and ensure the delivery of appropriate clinical care by the CNA staff.
  • Ensure a safe environment for patient population.
  • Notify attending physician and family or responsible party of any significant change in patient's health condition.
  • Maintain quality assurance logs for blood glucometer machines and other equipment necessary for quality nursing care.
  • Perform other duties as assigned by supervisor.
Requirements
  • Graduation from an accredited Nursing School and a current and valid license in the applicable State.
  • Minimum of one-year supervisory experience in a long-term or healthcare setting is preferred.
  • Ability to read, write, and speak the English language.
