Job Summary:

The LVN Charge Nurse will play a vital role in ensuring the delivery of high-quality care to our residents. This position requires a strong leader who can effectively manage and coordinate the care of our residents, while also maintaining a safe and supportive environment.

Key Responsibilities:
  • Assist in the development and implementation of individualized care plans for residents
  • Conduct regular rounds to assess resident condition and identify areas for improvement
  • Administer medications as prescribed and maintain accurate records
  • Collaborate with the healthcare team to ensure seamless care transitions
  • Monitor and report any changes in resident condition to the Director of Nurses and/or Administrator
  • Oversee and supervise LVN staff to ensure high-quality care delivery
Requirements:
  • Active Licensed Vocational Nurse (LVN) license in the state of Texas
  • Experience working in a skilled nursing facility preferred
  • Strong leadership and communication skills
  • Ability to work effectively in a fast-paced environment
