Clinical Physician Assistant

SUMMARY:
The Clinical Physician Assistant holds essential clinical duties, focusing on both preventive and therapeutic measures to enhance patient health care. This role includes making referrals for more specialized care as necessary. The Physician Assistant is accountable to the Chief Medical Officer (CMO) and the Chief Operations Officer (COO).

PRIMARY DUTIES AND RESPONSIBILITIES:
- Health Care Services:
- Maintain accurate chart documentation in accordance with clinic standards, utilizing SOAP charting and specific forms for referrals and test orders. Document both acute and chronic conditions along with medication management.
- Conduct follow-ups on designated disease categories and evaluate no-show patients.
- Review laboratory, x-ray, and referral documents prior to filing.
- Coordinate health education for patient follow-ups on specific health issues.
- Adhere to approved medical protocols and guidelines for treatment.
- Participate in on-call schedules as required.
- Provide phone triage on a rotational basis approximately every few months during weekends or evenings.
- Medical Records Management:
- Timely record and dictate notes for each patient visit using the SOAP format.
- Comply with Medical Records guidelines for maintaining charts.
- Ensure accurate data entry on encounter forms and review all medical records, lab results, and patient data before filing.
- Consultation and Referrals:
- Engage in consultations with the Medical Director for issues beyond standard procedures and refer to physicians as necessary.
- Facilitate referrals to other providers as indicated, including health education referrals per protocols.
- Team Collaboration:
- Contribute to the clinician peer review process.
- Participate in Continuous Quality Improvement (CQI) meetings and discussions as requested.
- Maintain interaction with referral resources.
- Refer to support services for counseling and education when appropriate.
- Provide input to the Chief Medical Officer regarding resource development, staffing, facilities, equipment, and supplies.
- Attend monthly General Staff Meetings and Monthly Departmental Clinician Meetings.
- Achieve efficient productivity levels in accordance with health center standards.
- Engage in daily huddles with the care team, which includes the Medical Assistant, Front Support staff, and Clinician, to discuss scheduling and pre-visit planning for optimal patient care.
Minimum Qualifications:
Education:
- Graduation from an accredited Physician Assistant Program.
Licensure:
- Possession of a valid California Physician Assistant License.
Certifications:
- Current CPR/BLS certification is required.
Skills:
- Bilingual in English and Spanish is preferred.
- Interest in promoting individual acute, chronic, and preventive care programs.
- Ability to foster positive interactions with patients and staff.
- Alignment with and support for the health center's mission.
Physical Requirements:
- Capability to lift up to 20 pounds and push up to 50 pounds (on wheels).
- Adequate hearing to auscultate blood pressures and communicate effectively with staff and patients.
- Vision sufficient to read medical documents and a computer screen accurately.
- High manual dexterity is essential.
- Ability to perform physical tasks such as bending, squatting, and reaching.
