Position Overview

SUMMARY:

The Advanced Practice Nurse holds essential clinical duties, focusing on both preventive and therapeutic interventions to enhance patient health outcomes. This role involves making referrals for more specialized care and operates under the supervision of the Chief Medical Officer (CMO) and Chief Operations Officer (COO).

KEY RESPONSIBILITIES:

PRIMARY DUTIES:

  1. Patient Care Services:
  2. Document patient interactions in accordance with clinic standards using SOAP methodology, including specific forms, referrals, and test orders.
  3. Conduct follow-ups on designated health conditions and manage no-show cases.
  4. Review lab results, imaging, and referral documents prior to patient record filing.
  5. Coordinate health education for patients addressing specific health issues.
  6. Adhere to established medical protocols and guidelines for treatment.
  7. Participate in on-call rotations.
  8. Provide telephone triage on a rotational basis approximately every few months during weekends or evenings.
  9. Medical Documentation:
  10. Timely record and dictate notes for each patient visit using the SOAP format.
  11. Maintain compliance with medical record-keeping standards.
  12. Ensure accurate data entry on encounter forms and review all medical records, lab results, and patient information before filing.
  13. Consultation and Referrals:
  14. Engage with the Medical Director for issues that exceed standard procedures and consult with physicians as necessary.
  15. Coordinate referrals with the Medical Director.
  16. Facilitate referrals to other healthcare providers as needed, including health education referrals per established protocols.
  17. Collaboration Expectations:
  18. Participate in peer review processes among clinicians.
  19. Engage in Continuous Quality Improvement (CQI) meetings and discussions as requested.
  20. Maintain connections with referral resources.
  21. Refer patients to support services for counseling and education when appropriate.
  22. Contribute insights to the Chief Medical Officer regarding resource development, staffing, facilities, equipment, and supplies.
  23. Attend monthly General Staff and Departmental Clinician Meetings.
  24. Meet productivity standards as outlined by the health center.
  25. Engage in daily huddles with the care team, which includes Medical Assistants and support staff, to discuss patient care and pre-visit planning.

Minimum Qualifications:

Education:

  • Completion of an accredited Nurse Practitioner Program.

Licensure:

  • Possession of a valid Nurse Practitioner License in California.

Certifications:

  • Current CPR/BLS certification is required.

Skills:

  • Bilingual in English and Spanish is preferred.
  • Willingness to promote programs addressing acute, chronic, and preventable care.
  • Ability to foster positive interactions with patients and colleagues.
  • Alignment with and support for the health center's mission.

Physical Requirements:

  • Ability to lift up to 20 pounds and push up to 50 pounds (on wheels).
  • Capability to hear adequately for auscultation and communicate effectively with patients and staff.
  • Vision sufficient to read clinical documents and operate necessary equipment.
  • High level of manual dexterity.
  • Ability to perform physical tasks including reaching, bending, squatting, and standing as required.
