Job Summary

The Ambulatory Care Nurse Clinician provides high-quality, patient-centered care to individuals with complex health needs in a fast-paced, dynamic environment.

Key responsibilities include:

  • Conducting comprehensive assessments to identify patient needs and develop individualized care plans
  • Implementing evidence-based interventions to promote optimal patient outcomes
  • Providing education and support to patients and their families to enhance health literacy and self-management skills
  • Collaborating with interdisciplinary teams to coordinate care and ensure seamless transitions

This role requires strong clinical expertise, excellent communication skills, and the ability to work effectively in a team-based setting.

Requirements
  • ADN or BSN degree
  • Current licensure as a Registered Nurse in the state of Kentucky
  • Basic computer skills and proficiency in electronic health records
