About the Role

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to join our team in providing private duty nursing care to our patients. As a Private Duty Nurse, you will be responsible for delivering skilled nursing care to patients in their homes, working under the supervision of a Registered Nurse. Your primary focus will be on providing one-on-one care to patients, assessing their needs, and developing and implementing individualized care plans to meet their unique needs.

Responsibilities
  • Implement and coordinate care plans in collaboration with the patient's healthcare team
  • Assess patient needs and develop individualized care plans to meet their unique needs
  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring
  • Document patient care and progress in a timely and accurate manner
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination
Requirements
  • Current, unrestricted LPN/LVN license in the state of practice
  • Current CPR certification
  • One year of experience as a Licensed Nurse in a clinical care setting
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
