Job Title: Aveanna Healthcare Private Duty Nurse LVNJob Summary

Aveanna Healthcare is seeking a compassionate and skilled Licensed Practical Nurse (LPN/LVN) to join our team as a Private Duty Nurse. As a Private Duty Nurse, you will provide one-on-one care to our patients in the comfort of their own homes, assisting them with transitioning home from the hospital and providing the continuous care needed to remain at home with their loved ones.

Responsibilities
  • Implement and coordinate patient care plans in accordance with physician orders.
  • Conduct patient assessments and coordinate care with the healthcare team.
  • Administer prescribed medication, treatments, and therapies as ordered by the physician.
  • Document all patient interactions, assessments, and care provided in a timely and accurate manner.
  • Provide education and training to patients and their families on disease management, self-care techniques, and prevention strategies.
  • Collaborate with the healthcare team to ensure seamless care transitions and address patient concerns.
Requirements
  • Current, unrestricted state license as a Licensed Nurse in the state of practice.
  • Current CPR certification.
  • Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care and policies and procedures.
  • One year of experience as a Licensed Nurse in a clinical care setting preferred.
