About the Role

We are seeking a skilled and compassionate Nurse Practitioner to join our Palliative Care Team in an inpatient setting. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals with serious illnesses.

Key Responsibilities
  • Provide symptom management recommendations and facilitate goals of care conversations for patients in medical, surgical, and ICU settings.
  • Participate in daily team rounds and communicate effectively with care teams to ensure seamless care coordination.
  • Document visits thoroughly and adhere to billing practices.
  • Contribute to the Palliative Care Committee and participate in education initiatives to promote palliative care awareness.
Requirements
  • Master's degree from an accredited program and current nurse practitioner license.
  • At least 2 years of experience, with a minimum of 1 year in hospice, oncology, or palliative care.
  • Commitment to palliative care philosophy and familiarity with regulatory requirements related to Medicare Part B.
  • Excellent communication and interpersonal skills, with the ability to work effectively with diverse patients, families, and healthcare teams.
