Associate Director of Nursing

The Associate Director of Nursing, Clinical and Community-Based Services will oversee a team of Transitional Care Nurses responsible for coordinating, managing, and disposing of patient referrals to the Certified Home Health Agency (CHHA).
Under the supervision of the Director of Nursing, Clinical and Community-Based Services, the Associate Director will focus on ensuring that the CHHA maintains a collaborative relationship with acute and post-acute care facilities across the health system.
Key Responsibilities- Develop and implement strategic plans to address the needs of referral sources, patients, and the CHHA to improve the timeliness and quality of home care service delivery.
- Participate in short- and long-range strategic planning and implement specific and targeted measures for census growth.
- Strengthen relationships with new and existing referral sources through daily or weekly visits to acute and post-acute care sites.
- Ensure that staff are counseled appropriately and timely regarding work performance, attendance, punctuality, quality in transitions of care, and productivity.
- Establish and maintain positive and productive relationships with acute and post-acute care stakeholders to support effective referral pathways and patient support during transition to home care.
- Work closely with Directors of Social Work and discharge planning teams to educate patients, families, and caregivers about home care regulatory guidelines, safe discharge planning, payer limitations, and restrictions.
- Develop and review metrics that address census growth, TCN productivity, and work performance.
- Oversee the daily functions and activities of a team of TCNs and provide one-to-one mentoring and coaching as needed.
- Licensed and currently registered to practice as a Professional Nurse in New York State.
- A Master's Degree in Nursing Administration, Education, or an equivalent field and a minimum of six years of satisfactory nursing experience, of which four years shall have been in a supervisory, teaching, or administrative capacity.
- A Baccalaureate Degree in Nursing and seven years of satisfactory nursing experience, five years of which shall have been in a supervisory or teaching capacity.
- A satisfactory equivalent combination of education and experience.
- Incorporate knowledge of cultural differences through effective instructional interactions.
- Comprehend principles of adult learning and incorporate them in formal and informal educational activities.
- Remain aware of current issues and trends in healthcare and translate them to the clinical team.
- Practice accountability and responsibility in all activities.
