Position Summary
The Patient Care Navigator serves as the primary contact for patients following their initial diagnosis. This role is crucial in educating patients and their families about their conditions, coordinating care, advocating for necessary support services, and addressing any barriers to optimal care.

The Navigator fosters a supportive relationship with patients and collaborates with healthcare team members to ensure a patient-centered experience throughout the treatment journey. This includes coordinating information for patients and their families to facilitate informed decision-making and acting as a liaison between treatment programs and referring physicians.

Key Responsibilities
  • Conducts outreach to ensure that patients are contacted during critical moments.
  • Introduces navigation services to streamline care and identify additional resources for patients and families.
  • Initiates contact with newly identified patients promptly and completes needs assessments in a timely manner.
  • Educates patients and families on disease specifics, treatment options, and available clinical trials to empower informed decisions.
  • Develops and coordinates individualized care plans based on patient needs, preferences, and values, updating as necessary.
  • Identifies and facilitates access to community support services and resources based on healthcare team recommendations.
  • Schedules and coordinates appointments and services, ensuring smooth transitions in care.
  • Advocates for patients and implements survivorship initiatives.
  • Ensures compliance with established policies and procedures to maintain patient safety.
  • Stays informed on advancements in the field and integrates knowledge into practice; participates in relevant meetings and boards.

Qualifications

Education and Licensure Required:
  • Bachelor's Degree in Nursing
  • Registered Nurse (RN) license

Minimum Experience:
  • Three (3) years of experience in patient care within the relevant field.
  • Experience in program development is preferred.

Preferred:
Experience in oncology is advantageous.

Minimum Knowledge, Skills, and Abilities:
  • Effective communication and advocacy skills with patients, families, and healthcare team members.
  • Ability to lead a team and collaborate to ensure high-quality patient care.
  • Strong critical thinking and problem-solving skills.
  • Comprehensive understanding of illnesses and treatment modalities.
  • Demonstrated sensitivity in coordinating care for diverse patient populations.
  • Knowledge of community resources and efficient access methods.
  • Ability to prioritize tasks and meet deadlines.
  • Skilled in assessing and coordinating patient needs.
