Care Coordination II: A key member of our healthcare team, responsible for delivering high-quality psychosocial services to patients. Key Responsibilities:

  • Provide crisis intervention and support to patients and their families.
  • Coordinate discharge care needs, ensuring a smooth transition for patients.
  • Collaborate with multidisciplinary teams to ensure comprehensive care.
  • Document patient information and care plans accurately and efficiently.

Requirements: The ideal candidate will possess excellent communication and interpersonal skills, with the ability to work effectively in a fast-paced environment. A strong understanding of psychosocial services and care coordination principles is essential.
