The Health Services Coordinator plays a vital role in enhancing the well-being of individuals within COMHAR's Outpatient Programs by managing and facilitating care.
This position is essential in connecting new clients to outpatient services through the intake process, providing necessary information and education, linking individuals to vital resources, and fostering communication with partner agencies, all while motivating clients to reach their recovery objectives.
Key Responsibilities:
- Deliver information, education, and encouragement throughout various stages of recovery, including pre-recovery engagement, recovery initiation, stabilization, and ongoing maintenance.
- Facilitate communication with primary care and other medical providers to ensure integrated healthcare.
- Maintain collaborative relationships with community partners, acting as a liaison to streamline care by enhancing communication, obtaining necessary consents, addressing service barriers, and participating in relevant meetings.
- Conduct initial assessments and manage the enrollment process for individuals seeking services at the Welcome Center and Open Access points.
- Engage in outreach initiatives, including mobile outreach, to connect individuals with suitable services.
- Proactively identify and offer connections to community resources that support the individual's recovery journey, including medical providers, recovery communities, educational and vocational resources, and professional services.
- Collaborate with individuals and treatment teams (including therapists, psychiatric practitioners, and case managers) to address recovery planning needs related to resources or care coordination.
- Assist individuals in crisis within behavioral health settings, which may involve accompanying them to crisis response centers or providing necessary documentation for crisis services.
- Ensure timely completion of required documentation, including care coordination encounters in the electronic health record.
- Support therapists with outreach and case management as needed.
- Adapt to a flexible schedule that accommodates evening and weekend hours to meet individual needs.
- Provide assistance across multiple outpatient programs, including Enhanced Outpatient and Specialized Services.
- Perform additional duties as assigned by supervisors or program leadership to enhance care coordination and integrated services.
Required Skills:
- Basic understanding of clerical procedures and systems, including recordkeeping.
- Ability to work both independently and collaboratively.
- Ability to follow instructions in various formats.
- Problem-solving skills in standardized situations.
- Respect for each individual's unique recovery journey, providing person-centered and trauma-informed support.
- Strong organizational capabilities.
- Familiarity with the recovery and treatment system.
- Flexibility in managing changing priorities in a dynamic environment.
- Professionalism and ethical service at all times.
- Proficiency in computer use, including electronic health records and Microsoft Office.
Physical and Work Environment:
The physical demands of this role require regular sitting, talking, and listening, with occasional standing and light lifting. The work environment is primarily within a fast-paced outpatient clinic, with meetings potentially occurring at various locations.
Education and Experience Requirements:
- Bachelor's degree in social work, counseling, psychology, sociology, or a related field.
- A minimum of 2 years of experience in health or human services, preferably in behavioral health or substance use.
- Valid clearances as required by state regulations.
- Current certification in recognizing and reporting child abuse.
