Job Summary: We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Bridge Senior Living. As a LPN, you will play a vital role in implementing and coordinating the delivery of care to our residents.
Key Responsibilities:
- Assess, plan, implement, and evaluate resident care plans to ensure the highest level of care.
- Provide frequent rounds to ensure continuity of care and document all pertinent care information.
- Collaborate with physicians and care associates to deliver comprehensive care.
- Maintain ongoing communication with residents, families, and physicians regarding resident care.
Requirements:
- First Aid & CPR Certified
- At least 1 year of related work experience
