LPN - Family Practice Nurse

We are seeking a highly skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Excela Health. As an LPN in Family Medicine, you will play a vital role in providing high-quality patient care and contributing to the success of our healthcare team.
Key Responsibilities- Direct Patient Care: Provide direct patient care activities, such as lab tests, treatments, and medication administration, under the direction of the Nurse Manager, RN Supervisor, RN, and/or physician.
- Clinical Activities: Carry out clinical activities, quality control activities, performance improvement initiatives, and other tasks in support of the Physician Practice as assigned.
- Essential Job Functions: Deliver direct and indirect patient care in accordance with the LPN Practice Act, hospital and/or physician practice policies, procedures, protocols, and other professional standards of care.
- Collect Patient Health Care Data: Collect patient health care data, including vital signs, weight, chief complaint, and pain assessment, focusing on the physical, psychosocial, age, cultural, and spiritual needs of the patient.
- Administer Medications: Administer medications/immunizations using appropriate routes and techniques specific to the age of the patient.
- Document Patient Care: Comply with completeness, accuracy, and timeliness of documentation.
- Provide Education: Provide education and instruction, within their scope of practice, to patients and families.
- Graduate of an Accredited School of Practical Nursing: Must be a graduate of an accredited School of Practical Nursing.
- Current Licensure: Must be currently licensed to practice as a Licensed Practical Nurse in the State of Pennsylvania.
- Basic Life Support (BLS) Certification: Must have current Basic Life Support (BLS) certification approved by the American Heart Association (CPR and AED) program.
- Act 33 and Act 34 Clearances: Must have current Act 33 and Act 34 clearances.
- Act 73 FBI Clearance: Must have current Act 73 FBI clearance.
This position requires the ability to work in a fast-paced environment and to prioritize tasks effectively. The LPN must be able to lift up to 50 pounds and to stand for long periods of time. The position also requires the ability to work in a team environment and to communicate effectively with patients, families, and healthcare team members.
