About the Role:

We are seeking a skilled Physical Therapist Assistant to join our Home Care department at MENTOR Adult Day Health. As a Physical Therapist Assistant, you will work under the supervision of a physical therapist to provide high-quality care to patients in their homes. Your primary responsibility will be to assist patients in achieving maximum independence, while maintaining professional standards and regulatory compliance.

Key Responsibilities:

  • Provides direct physical therapy treatment to patients according to instruction of Physical Therapist, and in accordance with the written plan of care.
  • Observes patient and reports to Physical Therapist and department personnel the patient's reaction to treatment and any changes in patient's condition.
  • Instructs patients, family and/or caregiver in proper techniques to assist patient in achieving maximum level of independence.
  • Maintains necessary records, progress notes, and conference notes, which are incorporated into the patient's clinical record within established time frame.
  • Maintains professional standards, compliance with State, Federal (CMS conditions of participation) and Joint Commission regulations, and infection control protocol by participating in in-services, staff meetings, process improvement teams, continuing education seminars when available, and adhering to the expectations of Sevita Health – standards of care guidelines.

Requirements:

The ideal candidate will have a High School Diploma or equivalent, one year of experience in a human services or related field, and a current PTA license in the state of work. You should also possess a good working knowledge of physical therapy equipment and therapeutic exercise treatments, as well as a self-motivated and detail-oriented nature with ability to multi-task. Additionally, a reliable, responsible, and caring nature, along with a commitment to the company's mission and values, is essential for this role.
