Clinical Pharmacist, Advanced Therapeutics Specialist

The Advanced Therapeutics Clinical Pharmacist provides patient-centered clinical pharmacy services, including comprehensive medication reviews, adherence monitoring interventions, and targeted medication reviews, to optimize clinical, economic, and patient outcomes. This role involves designing and implementing targeted medication and disease management programs to improve patient safety and achieve positive clinical and financial outcomes.
Key Responsibilities:
Provides medication management services in a collaborative manner, including reviewing patient medication history, educating patients on drug therapy, establishing desired outcomes, developing/ modifying therapeutic plans, determining alternatives for achieving outcomes, developing drug monitoring plans, monitoring patient outcomes, documenting medication-related issues in the electronic health record, interpreting laboratory data, reviewing patient medication profiles, monitoring pharmacokinetics of drug therapy, and monitoring for potential adverse drug-drug, drug-lab, drug-disease state, and drug-food interactions.
Maintains and updates patient profiles with demographics, diagnosis, allergies, and current medications.
Monitors drug therapy to evaluate appropriateness of use, dose, dosage form, regimen, route, therapeutic duplication, and drug interaction.
Provides drug information to healthcare professionals and patients.
Documents clinical interventions and activities in a timely manner, including progress notes in the EHR, annual comprehensive medication reviews with written summaries in the CMS's standardized format, and patient-specific documents, such as medication-related action plans and personal medication records.
Counsels patients on discharge medications as appropriate.
Performs clinical review and verification of medication orders.
Performs medication order entry when needed.
Discusses medication order clarifications with the prescriber, documents interventions, and communicates medication order changes as needed.
Facilitates medication discharge planning and smooth transition to alternate sites of care and access to an ambulatory pharmacy.
Maintains accurate and proper patient medication profiles, monitoring to ensure the proper choice of medication, dosage form, strength, length of therapy, and interaction potential of the prescribed medication.
Requirements:
Doctor of Pharmacy (Pharm.D.) degree; OR Bachelor of Science in Pharmacy (BSPharm) degree with 3 years hospital experience.
Licensure with the Massachusetts Board of Registration in Pharmacy.
Preferred:
ASHP Accredited Residency in Pharmacy Practice (PGY1) is recommended.
Pharm.D. degree with minimum of 1 year of hospital experience.
