Key Responsibilities- Conduct thorough assessments of patients' social and economic needs, identifying barriers to discharge and developing effective plans to address them.
- Collaborate with interdisciplinary teams, including physicians, nurses, and other healthcare professionals, to ensure comprehensive care and coordination.
- Develop and implement discharge plans that meet patients' unique needs, taking into account their preferences, strengths, and limitations.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care.
- Identify and report avoidable delays and variances in care, contributing to continuous quality improvement.
- Stay up-to-date with the latest treatment theories, practices, and techniques, applying this knowledge to provide exceptional care.
- Licensed Master's Social Worker (LMSW) certification, with eligibility for American Case Management Certification (ACM) or Case Management Certification (CCM).
- Three years of acute hospital care or social work experience, with a strong background in case management and discharge planning.
- Basic Life Support (BLS) certification as a Healthcare Provider.
