Key Responsibilities
  • Service Plan Development: Collaborate with consumers and guardians to develop and implement personalized service plans that meet their unique needs and goals.
  • Team Coordination: Schedule and chair team meetings to ensure seamless coordination of services and effective communication among team members.
  • Program Vacancy Management: Coordinate the development of personal futures plans and manage waiting lists for potential consumer vacancies.
  • Regulatory Compliance: Monitor service sites to ensure compliance with contractual, legal, and regulatory requirements, and implement corrective actions as needed.
  • Incident Reporting and Investigation: Monitor incident reports, investigate incidents of abuse, neglect, or mistreatment, and implement plans of correction to prevent recurrences.
  • Service Delivery and Training: Monitor and implement service plans, provide training to staff, and ensure compliance with regulatory requirements.
  • Fiscal Management: Manage fiscal spending within approved budget limits, including household expenses, vehicle expenses, personnel costs, and consultant services.
  • Risk Management: Monitor worker's compensation and unemployment claims, and take proactive steps to reduce claims and minimize risk exposure.
Requirements

To be successful in this role, you will need to possess excellent communication and organizational skills, as well as the ability to work effectively in a team environment. You will also need to be familiar with regulatory requirements and have experience in service coordination and management.
