Care Coordinator

Key Responsibilities:- Service Plan Development: Collaborate with consumers and guardians to develop personalized service plans that meet their unique needs and goals.
- Team Coordination: Schedule and chair team meetings to ensure effective communication and coordination among team members.
- Regulatory Compliance: Monitor and ensure that all service sites deliver services in accordance with contractual, legal, and regulatory requirements.
- Incident Reporting: Investigate and address incidents of consumer abuse, neglect, or mistreatment in a timely and thorough manner.
- Training and Development: Supervise the delivery of service plans and personal futures plans, and ensure that all written training programs are implemented and revised as needed.
- Fiscal Management: Maintain fiscal spending within approved budget limits and monitor worker's compensation and unemployment claims.
