Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team at Greater Portland Health. As a key member of our healthcare team, you will play a vital role in delivering high-quality patient-centered care to our diverse patient population.
Responsibilities- Assess, diagnose, and treat patients with a range of medical conditions, including mental health and substance use disorders.
- Develop and implement individualized treatment plans in collaboration with patients, families, and other healthcare professionals.
- Provide education and support to patients and their families to promote healthy lifestyles and disease prevention.
- Collaborate with other healthcare providers to ensure seamless care coordination and transitions.
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
- Graduate of an accredited Nurse Practitioner Program with a Maine State license to practice medicine and a valid DEA number.
- At least two years' experience as a Nurse Practitioner, with a strong preference for those with board certification by the American Academy of Nurse Practitioners or American College of Nurse Practitioners.
- Ability to be credentialed at assigned hospitals and contractible on Greater Portland Health insurance plans.
- Must not be excluded from participation in federal health care programs.
