Palliative Care Social Worker

Job Summary

We are seeking a skilled Palliative Care Social Worker to join our team at LifeBridge Health. As a Palliative Care Social Worker, you will play a critical role in ensuring that our patients and their families receive comprehensive, family-oriented care in the comfort of their own homes.

Key Responsibilities

  • Assess patients' and families' psychosocial, environmental, and financial needs, and develop a plan of care to address these needs.
  • Provide counseling services to patients and their families, offering support and guidance throughout the dying and grieving process.
  • Collaborate with other healthcare professionals to coordinate care and ensure seamless transitions between settings.
  • Develop and maintain relationships with community and agency homecare providers to ensure access to necessary services.

Requirements

  • Master's Degree in Social Work from an accredited institution.
  • Minimum of three years of clinical experience, with one year in a community health setting preferred.
  • LCSW or LCPC licensure required in Maryland and Pennsylvania.
