Hospice Social Worker Opportunity

LifeBridge Health is seeking a compassionate and skilled Hospice Social Worker to join our team. As a Hospice Social Worker, you will play a vital role in ensuring that our patients and their families receive the highest level of care and support during their end-of-life journey.



Job Summary

The Hospice Social Worker will be responsible for assessing patients' and families' psychosocial, environmental, and financial needs, and developing and implementing a plan of care in collaboration with the patient, family, and other caregivers. You will also provide counseling services to help patients and families cope with terminal illness, and coordinate with community and agency homecare referrals for services.



Requirements

  • Master's Degree in Social Work from an accredited school
  • Minimum of three years of clinical experience, with one year in a community health setting preferred
  • LCSW certification (preferred)
