Job Summary


As a Social Worker with LifeBridge Health, you will play a vital role in ensuring that patients and their families receive comprehensive, family-oriented care in the comfort of their own homes. Your expertise will be instrumental in assessing patients' and families' psychosocial, environmental, and financial needs, and developing personalized plans of care that address their unique challenges. You will work collaboratively with patients, families, and other caregivers to provide counseling services, case management, and support during the dying and grieving process. With a focus on delivering high-quality, patient-centered care, you will be an integral part of our team at LifeBridge Health.

Responsibilities
  • Assess patients' and families' psychosocial, environmental, and financial needs
  • Develop and implement personalized plans of care
  • Provide counseling services and case management
  • Collaborate with patients, families, and other caregivers to ensure comprehensive care
  • Document patient information and progress

Requirements
  • Master's Degree in Social Work from an accredited program
  • Minimum of three years of clinical experience, with one year in a community health setting preferred
  • LCSW certification (preferred)
